   We will call you and inquire about certain specifics of your life style to see if you are a good fit for the treatment.
   
2. Once accepted as a candidate, please refer to the following pages to have a better understanding about the treatment:

  Please read carefully possible about adverse effects that may appear during process of the treatment. It is worth mentioning that even though some of the adverse effects may look serious, but 99% of them will be eliminated under proper guidance of further treatment.

  3. Please read the “AAC Arbitration Agreement“. If you choose to continue and receive our moxibustion service, please sign and date after ” PATIENT SIGNATURE X “, located at the bottom of the last two pages; or a copy will be offered at the meeting of service for you to sign.
